Successful Heart Rate Management of Atrial Fibrillation Due to Thyroid Storm Using Intravenous Amiodarone

چکیده
Intravenous amiodarone is commonly used for heart rate control in patients with atrial fibrillation who are at risk of hemodynamic instability; however, its administration severe thyroid dysfunction often avoided because known toxicity. Furthermore, the existing literature focusing on this topic limited. Here, we present a case storm, fibrillation, and reduced left ventricular systolic function, whose rapid was successfully controlled using intravenous conjunction antithyroid drugs. An 83-year-old man presented to emergency department worsening dyspnea over 1 week diagnosed impending storm failure ejection fraction. Treatment thiamazole, potassium iodine, hydrocortisone, landiolol, short-acting beta-blocker, initiated. Although patient initially had sinus rhythm, he developed response associated low blood pressure. increased dose landiolol further lowered pressure but failed rate. Therefore, initiated, leading reduction rate, without causing instability, an eventual return rhythm. There no recurrence discontinued 48 h after administration. The final diagnosis caused by painless thyroiditis. He transferred long-term care hospital day 69. Theoretical evidence suggests that combination drugs may mitigate potential thyrotoxicity, rendering it favorable choice fibrillation. Hence, be considered if initial therapy beta-blockers fails. However, research clinical studies required validate safety efficacy approach. J Endocrinol Metab. 2023;13(3):126-133 doi: https://doi.org/10.14740/jem887

We are presenting a case illustrating atrial fibrillation (AF) following the use of methylprednisolone in a patient with pelvic and femur fracture. A 48- year- old man with no significant past medical history, was admitted to the emergency department after injury in a car accident. He suffered a multiple bone fracture with chief complaints of pain and shortness of breath. He was transfer...
